CARE PLAN

Your care team is comprised of a number of individuals equipped with the skills and knowledge to ensure that you receive the highest quality care possible. 

 

  • You are the most important member of the care team. Together, this team can work on addressing your specific needs and ensuring the highest quality of care is provided. 

 

  • Your doctor is responsible for diagnosing kidney failure, presenting you with the available treatment options, and prescribing your dialysis treatment and medications.

 

  • Your nurse is responsible for administering your dialysis treatment.  Nurses are an excellent source of information when it comes to kidney failure, treatment options, medications, and any additional steps you can take to stay as healthy as possible.

  

  • Your social worker can answer any questions you may have regarding insurance, medical bills, transportation to and from the dialysis unit, and any benefits you may be entitled to.  If you have any stress or concerns regarding chronic illness and long-term dialysis, your social worker is available to help you.

 

  • Your dietitian will assist you with your restricted diet through education and meal planning.

 

  • Technicians are responsible for maintaining the optimal working condition of our dialysis equipment and assisting nurses with dialysis treatment.

 

  • Your unit secretary can assist you with scheduling appointments and refer you to the appropriate person(s) to address any questions or concerns you may have.
